2012 CDC and HSA School Health Profiles

healthy school pic In an effort to better understand the factors influencing student health, the Office of the State Superintendent of Education (OSSE) requests that faculty, staff and parents complete the Healthy Schools Act School Health Profiles (HSA SHP) and the Centers for Disease Control and Prevention School Health Profiles (CDC SHP). Although they share similar titles, the two profiles are not the same. Taken together, the HSA SHP and the CDC SHP give a comprehensive picture of the factors influencing student health in schools.

Healthy Schools Act School Health Profile

Under Section 602 of the Healthy Schools Act of 2010, each public school and public charter school within the District of Columbia is required to complete and submit the HSA SHP form to OSSE on or before February 15th of each year. You’ll find information here on the website and publications available to parents for pick up at the main office. Once completed, School Without Walls will receive an electronic copy for our records.

CDC School Health Profile

Through a partnership with the Centers for Disease Control and Prevention (CDC), the OSSE works to foster coordinated school health policies and programs for our youth.  As part of this partnership, they are conducting surveys of principals and lead health education teachers to assess the status of school health policies and practices at the middle and high school levels.

The data from the CDC SHP will not be reported about individual schools, principals, or teachers. Please complete the CDC School Health Profile questionnaires no later than March 30, 2012. When completed, you can print copies of the completed profiles.

Completion Instructions

Both the CDC and the HSA School Health Profiles should be completed and submitted online by each school. Attached are detailed instructions to complete the CDC SHP and HSA SHP forms, including the following:
